The Code requires senators to complete and file with the Office of the Senate Ethics Officer various forms:

Confidential Disclosure Statement (PDF ^NEW)
Confidential Disclosure Statement (Word)
Statement of Gifts and Other Benefits
Statement of Senator’s Sponsored Travel
Statement of Material Change
Public Disclosure Summary

The Code also requires the Senate Ethics Officer to prepare a Disclosure Summary which is available for examination by the public in the Office of the Senate Ethics Officer.
